The radiographic posterior fat pad (PFP) sign is one of the frequently referenced indirect signs of an occult elbow fracture. This sign can be falsely negative if the sign is subtle, and can be falsely positive when the position of the elbow is not flexed at 90 degrees.

The elbow fat pads are situated external to the joint capsule. On a true lateral radiograph, the normal anterior fat pad is seen as a radiolucent line parallel to the anterior humeral cortex; and the posterior fat pad is invisible.
Fat Pads. On a normal elbow x-ray, only a small stripe of an anterior fat pad should be visible. No posterior fat pad should be seen. Injuries around the joint can produce a joint effusion which will displace the fat pads making them more visible. The anterior fat pad protrudes more and looks pointy; this is what is recognized as the sail sign.

This is an invaluable soft tissue finding in cases of intra-articular injury of the elbow. Fat is normally present within the joint capsule of the elbow, but outside the synovium.
Distension of the elbow joint (which in the setting of trauma is assumed to be from blood from an intraarticular fracture) will displace the fat pads from their normal location and have characteristic radiographic appearances. The anterior fat pad is normally identified on a true lateral radiograph as a triangular radiolucency abutting the joint.
